In the past, CDC has recommended to only accept patient-reported history for influenza and pneumococcal polysaccharide vaccines. Is the recommendation to not accept a patient-reported history of pneumococcal conjugate vaccine?

CDC’s “General Best Practices for Immunization” still states that a patient’s self-report of a history of vaccination without any documentation can generally be accepted as proof of vaccination only for influenza and pneumococcal polysaccharide vaccines. The ACIP recommendation for administering a primary series of inactivated polio vaccine to an adult also indicates that adults may generally be presumed to have had a primary series of polio vaccine unless there is a specific reason to believe that they have not been vaccinated against polio.
