Ask the Experts: Immunize.org Answers Questions about Universal Adult Hepatitis B Screening and Vaccination
As an additional service to IZ Express readers, we periodically publish special editions such as this one, providing you with new and updated Ask the Experts questions and answers from Immunize.org experts. This issue includes 17 Q&As related to universal adult hepatitis B screening and vaccination guidelines. These Q&As also are available in a print-ready format from Immunize.org at www.immunize.org/catg.d/p2080.pdf. Download and share these practical Q&As with members of the healthcare team working to implement adult hepatitis B screening and vaccination into clinical practice.

You can find all of these Q&As and more than a thousand other Q&As about vaccines and vaccine administration on our "Ask the Experts" main page at www.immunize.org/askexperts.

Immunize.org's team of experts includes Kelly L. Moore, MD, MPH (team lead), Carolyn B. Bridges, MD, FACP, and Iyabode Beysolow, MD, MPH.
Universal Adult Hepatitis B Screening and Vaccination Questions
Q: Which adults should be vaccinated for hepatitis B?
Q: Which adults should be screened for hepatitis B?
Q: Why should healthcare professionals focus on vaccinating all adults against hepatitis B now?
Q: Where can I find information about hepatitis B vaccination and post-vaccination testing for people who work in healthcare?
Q: In light of the 2023 CDC hepatitis B screening recommendations, should a healthcare worker who needs to be tested for anti-HBs to confirm immunity after HepB vaccination have the triple panel screening test (anti-HBs, anti-HBc, HBsAg) instead of an isolated anti-HBs?
Q: People with risk factors who are 60 and older should be vaccinated and other people older than 60 may be vaccinated. How does CDC intend clinics to handle people 60 and older?
Q: If the adult patient hasn’t been screened for hepatitis B yet, should you screen them before you decide about vaccinating?
Q: Why does CDC recommend HepB vaccination of adults in settings (such as most pharmacies) where triple panel hepatitis B screening is not available?
Q: If you are going to give an adult HepB vaccine and draw the screening hepatitis B triple panel at the same visit, does it matter which one you do first?
Q: What do I do for adult patients who don’t have records of HepB vaccination, but are sure they were vaccinated?
Q: Why should I do a triple panel screening test for hepatitis B status on adults we know were vaccinated?
Q: Do adults who were properly vaccinated need to be revaccinated if they have a negative surface antibody (anti-HBs) result on their triple panel screening test for hepatitis B?
Q: Does it matter which adult HepB vaccine I use? Are there differences among the recommended products?
Q: What do I do if an adult patient has documentation of one dose of HepB vaccine of unknown type a few years ago, but never finished the series?
Q: What should I do if an adult patient needs screening for hepatitis B, but recently received the first dose of HepB vaccine elsewhere?
Q: What do I do with an adult whose triple panel hepatitis B screening result shows only the total core antibody (anti-HBc) is positive?
Q: Adults go to many different healthcare providers over time. HepB is a vaccine series most only need once. How do we avoid the problem of lost records for adult patients we vaccinate now?
Q: A healthy young adult patient who received the HepB vaccine series before college recently had a triple panel screen and discovered that he had chronic hepatitis B. Did the vaccine fail? What do I tell the patient?

Ask the Experts: Immunize.org Answers Questions about Rabies Vaccines and Scheduling or Documenting Vaccinations
As an additional service to IZ Express readers, we periodically publish special editions such as this one, providing you with new and updated Ask the Experts questions and answers from Immunize.org's experts. This issue includes 5 Q&As about rabies vaccination, and 6 Q&As about scheduling or documenting vaccinations.

Rabies Questions
Q: How common is rabies in the United States?
Q: What kind of vaccine is the rabies vaccine?
Q: What is the pre-exposure prophylaxis (PrEP) schedule for rabies vaccine?
Q: How does rabies post-exposure prophylaxis (PEP) differ from getting vaccinated before an exposure (PrEP)?
Q: Who should I contact if I have a patient who may have been exposed to a rabid animal?
Scheduling Vaccines Questions
Q: How can we quickly determine how to "catch up" children who have fallen behind on their shots?
Q: For the purpose of vaccine spacing, what constitutes a month: 28 days (4 weeks), 30 days, or 31 days?
Q: Is it necessary to start a vaccine series over if a patient doesn't come back for a dose at the recommended time, even if there's been a year or more delay?
Documenting Vaccination Questions
Q: In the past, CDC has recommended to only accept patient-reported history for influenza and pneumococcal polysaccharide vaccines. Is the recommendation to not accept a patient-reported history of pneumococcal conjugate vaccine?
Q: If a patient or parent refuses recommended vaccinations, is it necessary for them to sign a refusal form, or is the provider's documentation sufficient?
Q: Under the Health Insurance Portability and Accountability Act (HIPAA), we are unsure if we can share immunization information on our pediatric patients with staff in schools or daycare facilities.

Ask the Experts: As seen on social media
Immunize.org now regularly features both written and short video versions of clinical questions and answers (Q&As) from our popular Ask the Experts section on our social media channels: YouTube, Instagram, Facebook, Twitter, and LinkedIn. This issue introduces you to three of our new Ask the Experts videos, plus a selection of 7 Q&As recently shared on social media. Follow Immunize.org on your preferred social media channels for posts that make it easier for you to improve your vaccination practices a little every day.

Because COVID-19 vaccination recommendations and clinical guidance are updated frequently, Immunize.org reminds our readers to regularly visit CDC’s web page: Use of COVID-19 Vaccines in the United States.

You can also find these Q&As and more than a thousand other Q&As about vaccines and vaccine administration on our “Ask the Experts” main page at www.immunize.org/askexperts.

Immunize.org’s team of experts includes Kelly L. Moore, MD, MPH (team lead), Carolyn B. Bridges, MD, FACP, and Iyabode Beysolow, MD, MPH.
Video Q&As
Q: How late in the season can I vaccinate patients with influenza vaccine?
Q: What’s the difference between a contraindication and a precaution?
Q: We frequently see patients who are febrile or have an acute illness and are due for vaccinations. We are uncertain if we should withhold the vaccines or not. What do you advise?
Recent Social Media Q&As
Q: How does CDC define being “up to date" on COVID-19 vaccination?
Q: May I use Novavax COVID-19 protein subunit vaccine as a booster dose?
Q: What proof do I need for COVID-19 vaccination of a patient who says he is moderately or severely immunocompromised?
Q: Does CDC recommend restarting the hepatitis B vaccine series in the event the series is interrupted?
Q: Several physicians in our group have no documentation showing they received hepatitis B vaccine. They are relatively sure, however, that they received the doses many years ago. What do we do now?
Q: I'm a nurse who received the HepB series more than 10 years ago and had a positive follow-up titer (at least 10 mIU/mL). At present, my titer is negative (less than 10 mIU/ mL). What should I do now?
Q: If dose #1 of HPV vaccine was given before the 15th birthday and it has been more than a year since that dose was given, would the series be complete with just one additional dose?
