Users of IAC's free print
materials--please take our survey!
As many of you know, IAC makes
available a wide range of downloadable print materials on our Free Print
Materials web page located at
http://www.immunize.org/free
If you have ever downloaded, printed, or distributed one or more of IAC's
free print materials (excluding VISs), we ask that you take our online survey
located at http://www.immunize.org/surveyfree/?wherefrom=iacx
Please DO NOT take this survey if you have already filled out our (identical)
survey presented to visitors on the Free Print Materials web page, which was
available online between January 15 and February 3.
IAC's print materials include summaries of recommendations, screening
questionnaires, vaccine administration records, parent education handouts,
temperature logs, and viral hepatitis information, among others.
Our colleagues at CDC are asking us to find out precisely how these materials
have a role in the delivery of immunization services. The survey is designed
to obtain your feedback about the ways you use our printed items and how they
affect your immunization knowledge and practices, or your patient-education
efforts.
The survey also is an opportunity for you to tell us what types of materials
are most valuable to you and to indicate which pieces you find most useful.
Your input is very important to our continuing efforts to bring you accurate
and effective print materials.
If you would like to review the Free Print Materials web page, please go to
http://www.immunize.org/free
The survey is located at
http://www.immunize.org/surveyfree/?wherefrom=iacx
Thank you so much!
