PRODUCT DESCRIPTIONS
IAC offers four types of immunization products with individual products within each type. The four product types are (1) laminated versions of the CDC's 2009 immunization schedules (2) padded screening questionnaires, (3) immunization record cards, and (4) an immunization techniques training video in two formats. Details on each follow.

We offer two laminated versions of the CDC's 2009 immunization schedules:

  • Recommended Immunization Schedule for Children and Adolescents Ages 0 through 18 Years, United States 2009
  • Recommended Adult Immunization Schedule, United States 2009

A pair of 2009 laminated immunization schedules is a must for every exam room at your practice site. We offer them for two age groups. The child/teen immunization schedule is for people ages 0 through 18 years. It is adapted from CDC's "Recommended Immunization Schedules for Persons Aged 0 Through 18 Years, United States, 2009," which was endorsed by the Advisory Committee on Immunization Practices, American Academy of Pediatrics, and American Academy of Family Physicians. The adult schedule is for those older than 18. It is adapted from CDC's "Recommended Adult Immunization Schedule, United States, 2009," which was endorsed by Advisory Committee on Immunization Practices, American Academy of Family Physicians, American College of Obstetricians and Gynecologists, and American College of Physicians.

For the second year in a row, IAC has included a significant feature on the two laminated schedules. The laminated child/teen schedule includes a "Guide to Contraindications and Precautions to Commonly Used Vaccines in Children and Teens." The laminated adult schedule includes a "Guide to Contraindications and Precautions to Commonly Used Vaccines in Adults." IAC adapted the guides from "General Recommendations on Immunization: Recommendations of the Advisory Committee on Immunization Practices," which was published in the Morbidity and Mortality Weekly Report on December 1, 2006. IAC recently updated the two guides to reflect contraindications and precautions found in vaccine-specific recommendations that were published after December 1, 2006. Healthcare professionals will find the updated guides extremely valuable in making an on-the-spot determination about vaccinating any patient.

The new 2009 laminated schedules are coated in plastic to withstand heavy-duty use. They come complete with essential footnotes and are printed in color for easy reading. Each schedule has six pages (i.e., three double-sided pages), and when folded, measures 8.5" x 11".

To learn more about the schedules, to view images of each, or to order online or download an order form, visit www.immunize.org/shop

Prices start at $10 each for 1-4 copies and drop to $6.50 each for 5-19 copies. Discount pricing is available for 20 or more copies. For quotes on customizing or placing orders in excess of 999 schedules, call (651) 647-9009 or email admininfo@immunize.org

We offer two padded screening questionnaires for contraindications and precautions to vaccines:

  • Screening Questionnaire for Child and Teen Immunization
  • Screening Questionnaire for Adult Immunization

If you are you looking for a quick, easy, and thorough way for you and your patients to determine if they have contraindications and precautions to vaccination, look no further! IAC has two products--a padded Screening Questionnaire for Child and Teen Immunization and a second padded Screening Questionnaire for Adult Immunization--that solve the problem easily. Using these simple 1-page questionnaires, patients check boxes "yes" or "no" while waiting to be seen. Their answers are then ready for your review.

The questionnaires come in convenient tear-off pads of 100 sheets. The price per pad is economical (discounts for 2 pads or more), so you'll be able to keep pads at the receptionist's desk, the nurse's station, and in every exam room. Each pad comes with four reference sheets for health professionals, which are printed on heavy-weight paper. To learn more about the padded questionnaires, or to order online or download an order form, visit http://www.immunize.org/shop

Prices start at $16 each for one pad and drop to $12 each for two pads, $11 each for three pads, and $10 each for four pads. For quotes on larger quantities or customizing, call (651) 647-9009 or email admininfo@immunize.org

We offer three immunization record cards:

  • Adult Immunization Record Cards
  • Child and Teen Immunization Record Cards
  • Lifetime Immunization Record Cards

Our vaccination record cards include the popular adult card (more than 6 million sold) and two newer cards--one for recording child and teen vaccinations and another for recording lifetime vaccinations.

The cards offer health professionals a way to help any patient maintain a permanent record of their vaccinations. A patient-held card is handy for patients when they enter school, change healthcare providers, or travel abroad.

Each type of card is designed for a specific age group and lists all vaccines recommended for people in the age group. Features of the cards include the following:

  • All recently licensed vaccines: MCV, HPV, RV5, RV1, Tdap, zoster
  • A column to record the due date for the next vaccine dose
  • Space for patient's name and birth date on both sides (helpful when photocopying)
  • Space for patient I.D. number

The cards are printed on durable rip-, smudge-, and water-proof paper. Sized to fit in a wallet, they are brightly colored to stand out. To learn more about IAC's cards and to view images of each, or to order online or download an order form, visit http://www.immunize.org/shop

To request sample cards, email admininfo@immunize.org

Pricing starts at one box (250 cards) for $37.50. The first order of a 250-card box comes with a 30-day, money-back guarantee. Discounts are available for larger orders: two boxes, $35 each; three boxes, $32.50 each; four boxes, $30 each.

In recent years, many states, health plans, and corporate partners have placed large orders for IAC's record cards and had them customized with special colors, logos, and text. For quotes on customizing or placing orders in excess of 200 boxes, call (651) 647-9009 or email admininfo@immunize.org

The vaccine administration training video, "Immunization Techniques: Safe, Effective, Caring," comes in two formats:

  • DVD: "Immunization Techniques: Safe, Effective, Caring"
  • VHS videotape: "Immunization Techniques: Safe, Effective, Caring"

If you vaccinate children or adults, your practice needs this training video. Developed in 2001 by the California Department of Health Services Immunization Branch in collaboration with a team of national experts, the video is considered a vital immunization tool.

In a short 35 minutes, it teaches best practices for administering intramuscular (IM) and subcutaneous (SC) vaccines to infants, children, and adults. It is designed for use as a "hands-on" instructional program for new staff and as a refresher course for experienced vaccinators.

The video is available in DVD and VHS videotape formats. For information about the two formats and the print materials included with each, or to order online or to download an order form, visit http://www.immunize.org/shop

Both the DVD and VHS videotape are priced at an economical $10.50 per copy. For quotes on placing orders in excess of 500 copies, call (651) 647-9009 or email admininfo@immunize.org
